Drug logging Question

Hey all.

I’m in charge of the drug log at my hospital. We use paper ones. Not the AAHA books, but they’re styled similarly after them. As often as I can (usually 2-3 times a week, sometimes less if it’s crazy), I’ll audit the drug log. Make sure entries match with billing triggers. Sometimes there’s a drug we used, but forgot to log. In that case, I’ll log it with the current date and in the notes column write “missed entry from X date”.

I have two questions.

  1. What do you guys do when you find out a drug hasn’t been logged after the date it was administered?

  2. In addition, what do you usually write when a bottle is empty, but there is still supposed to be some volume in the log? Usually I’ll account it to hub loss if the number matches to <0.05ml per draw after doing some math.

I can’t switch to digital logging. Tried in the past.

Also, I’m in California.
